  • Use BLOCK LETTERS while filling the form to ensure clarity.
  • Ensure all mandatory fields are correctly filled – incomplete forms may be rejected.
  • Double-check your PAN, bank account details, and folio numbers for accuracy.
  • Signature(s) should match the one registered with the mutual fund and KYC records.
  • Select appropriate options wherever checkboxes are provided; do not leave any checkboxes blank unless explicitly stated as optional.
  • Nomination is strongly recommended for all individual folios. Kindly fill out the nominee details to ensure smooth transmission of assets.
  • If investing on behalf of a minor, ensure guardian details are accurately provided with valid proof of relationship and guardianship.
  • Strike out unused fields or sections clearly to prevent unauthorized additions or misuse.
  • Attach self-attested copies of required documents such as PAN card, KYC acknowledgment, and a cancelled cheque leaf (for bank details verification).
  • For joint holdings, ensure all holders sign where required.
  • Avoid overwriting. If a correction is necessary, strike through the error, write the correct information, and place a small signature next to the correction.
  • Read the scheme information document (SID) and key information memorandum (KIM) before investing.
  • Review the form thoroughly before submission to avoid corrections or rejections later.
